Appointments and Cancellations

To make an appointment, please call (404) 377-7711 and talk with our receptionist. She will make every effort to arrange a convenient time for you. If you are having any dental discomfort, please let us know as early in the day as possible so we can see you the same day.

If it is necessary to cancel an appointment, we ask the you give us as much notice as possible -- at least 24 hours -- so that your previously reserved time can be made available to another patient. Patients who consistently miss appointments without notice will not be rescheduled.

Appointment delays unfortunately do occur in our office occasionally. We realize that your time is also valuable, and we schedule appointments accordingly. Delays happen usually due to our efforts to work into our schedule a patient who has a dental emergency. We ask for your understanding if this should occur. We always provide this emergency service to all our patients.

Changes in Your Medical History

So that we can take every necessary precaution when providing excellent dental care, please notify us of any change in your medical history. Also, please inform us of any change of name, address, phone number, marital status, employment, or insurance coverage.

We request that our new patients complete a medical history form prior to your first visit. You may complete this form in our office prior to your visit, or if you prefer, you can download the form from our website, print it out and complete it, then bring it in with you.

From Chamblee, Dunwoody, Doraville, Roswell, and Alpharetta

Get on I-285 eastbound, heading towards I-85 and I-20. Take the fourth exit past I-85, US 78 Westbound, and merge onto the freeway. After approximately 1 mile, US 78 merges with Lawrenceville Highway as the freeway ends. Shortly after merging onto Lawrenceville Highway, turn left onto Church Street, and go approximately 1.7 miles to Commerce Drive. Turn Left on Commerce Drive, and then take a right onto North Candler Street at the next traffic light. Park in the visitor's parking deck for Commerce Plaza and Decatur Hospital, which will be on your right.

From Stone Mountain, Snellville, and Loganville

Get on US Highway 78, headed West towards Decatur and Atlanta. Continue through I-285, and merge onto Lawrenceville Highway at North DeKalb Mall. Turn left onto Church Street, and go approximately 1.7 miles to Commerce Drive. Turn Left on Commerce Drive, and then take a right onto North Candler Street at the next traffic light. Park in the visitor's parking deck for Commerce Plaza and Decatur Hospital, which will be on your right.

From Tucker, Lilburn, and Lawrenceville

Follow Lawrenceville Highway South through Tucker. After crossing I-285, go about 2.5 miles, passing North Druid Hills Road and North DeKalb Mall. Make a left on Church Street, and travel about 1.7 miles to Commerce Drive. Turn Left on Commerce Drive, and then Right at the first traffic light, which is North Candler Street. Park in the visitor's parking deck for Commerce Plaza and Decatur Hospital, which will be on your right.

From Decatur and Atlanta

Proceed Eastbound on Ponce De Leon, At the intersection with Scott Boulevard, bear Right onto West Ponce De Leon. Go approximately 1 mile to North Candler Street and turn left. Turn Left, and park in the visitor's parking deck for Commerce Plaza and Decatur Hospital, which will be on your left.
