Appointments and Cancellations

We request that our new patients complete a medical history form prior to your first visit. You may complete this form in our office prior to your visit, or if you prefer, you can download the form from our website, print it out and complete it, then bring it in with you.

So that we can take every necessary precaution when providing excellent dental care, please notify us of any change in your medical history. Also, please inform us of any change of name, address, phone number, marital status, employment, or insurance coverage.

To make an appointment, please call (404) 377-7711) and talk with our receptionist. She will make every effort to arrange a convenient time for you. If you are having any dental discomfort, please let us know as early in the day as possible so we can see you the same day.

If it is necessary to cancel an appointment, we ask the you give us as much notice as possible -- at least 24 hours -- so that your previously reserved time can be made available to another patient. Patients who consistently miss appointments without notice will not be rescheduled.

Appointment delays unfortunately do occur in our office occasionally. We realize that your time is also valuable, and we schedule appointments accordingly. Delays happen usually due to our efforts to work into our schedule a patient who has a dental emergency. We ask for your understanding if this should occur. We always provide this emergency service to all our patients.
